Medical Forms and Medication Cards

Why?

Scout camp is a wonderful experience, but it can also be physically demanding. Most Scouts get more exercise in a day at camp than they get in a week at home. (Evidenced by how quickly they hit the sack on Monday night) In order to best protect everyone who comes to camp, we require that all participants have visited the doctor. Examinations allow us to be sure of everyone's condition and aware of any special medical needs a Scout or Scouter may have.

The Health Officer will review each examination with its owner upon arrival at camp. Parents should know that it is our intent to use this information as a tool to help us better respond to potential situations. Your can help us by being as complete and honest as possible. Health information will not be used to restrict activity or single out Scouts.

What is Required?

EVERYONE ATTENDING CAMP MUST HAVE A CURRENT MEDICAL EXAMINATION FORM

BSA Annual Health and Medical Record must be completed and signed for all partipants at camp. Portions A and B must be completed for those attending low impact programs at camp lasting less than 72 hours.

For summer camp and all high adventure and high impact activities, section C must also be completed by a physician on an annual basis. Please note that anyone spending the night during summer camp is considered participating in this high impact program and must complete section C. This is required for all Scouts and leaders, even those who are not spending more than 72 hours in camp.

Camp work weekends may also fit the classification as high impact activites and require a physical examination, be sure to check with organizers before attending. Activities at Bear Paw will not require adherance to the height and weight restrictions, but some high adventure activites may fit these categories.

Any person spending the night at camp or participating in any camp activities must have a completed Annual Health and Medical Record on file. Boy Scout medical examination forms are available in the leader handbook, at any of the Bay-Lakes Council Offices, or download Medical Forms from our forms page.

Distribution of Medication

Wisconsin state law requires that all medications for Scouts at camp be kept by the health officer. (Accommodations are made for those cases where 24 hour emergency access to medication is required.) For each medication a card must be completed that has a doctor's signature dated within the last 12 months and instructions as well as a place to log each time the medication is dispensed. Scouts will be asked to turn in their medications and medication cards on arrival at camp. Medications should arive at camp in the original pharmacy container.
